Clinic KPIs Every Owner Should Track: A Guide for Clinics in Pakistan

Most clinic owners in Pakistan know two numbers by heart: how much cash came in today and how many patients were seen. Both matter, but neither explains why one month is profitable and the next is not, why a new doctor is busy but the clinic is not earning more, or why the waiting area is full while follow-ups quietly disappear. Clinic KPIs (key performance indicators) turn daily activity into a small set of numbers an owner can review every week. This guide lists the clinic KPIs every owner should track, how to calculate each one, what usually moves them, and how to collect them without spending evenings on spreadsheets.
Why track clinic KPIs at all?
A clinic is a business with a limited resource: doctor time and room time. KPIs show whether that time is used well and paid for. They also make conversations easier. Instead of "the clinic feels slow this month", an owner can say "new patients dropped by 20 percent and no-shows doubled on Saturdays", which points straight at a fix. Ten good numbers reviewed every Monday beat fifty numbers nobody looks at.
Patient KPIs
1. New patients per month
Count patients registered for the first time in the month. This is the clearest measure of whether marketing, referrals and the clinic's Google listing are working. Track it by source where possible (walk-in, referral, social media, Google, booking link).
2. Returning patient rate
Returning patients divided by all patients seen in the period. Aesthetic, dermatology, dental and physiotherapy clinics depend on returning patients far more than they depend on new ones. A falling returning rate is usually a follow-up problem, not a marketing problem.
3. Follow-up completion rate
Follow-ups that actually happened divided by follow-ups the doctors asked for. This needs follow-up dates recorded in the visit, not only on the paper prescription.
4. Patient satisfaction
A short, consistent feedback question after the visit (for example a QR code at reception) gives a trend that can be compared month by month. Ask every patient the same way; do not only ask the happy ones.
Appointment KPIs
5. No-show rate
No-shows divided by booked appointments. In many Pakistani clinics this sits well above what owners expect, especially for early-morning and Saturday slots. WhatsApp confirmations the day before usually move this number fastest. See how to reduce patient no-shows.
6. Cancellation rate and lead time
Late cancellations are almost as costly as no-shows. Track how many cancellations happen on the same day.
7. Doctor utilisation
Booked and seen hours divided by available hours for each doctor. Low utilisation means a doctor needs more patients or fewer hours; very high utilisation with long waits means the schedule is too tight.
Revenue KPIs
8. Revenue per doctor and per service
Which doctors and which services bring in the money? Many clinics discover that one or two procedures (laser, HydraFacial, root canal, physio packages) carry the month, which shapes marketing and pricing.
9. Average bill per visit
Total billed divided by number of visits. Watch it alongside discounts: an average bill that rises only because discounts fell is a different story from one that rises because of more procedures.
10. Discounts as a percentage of revenue
Discounts given at the counter add up quickly in PKR terms. A clear discount policy and recording every discount in the bill (not as cash adjustments) make this visible.
11. Outstanding payments
Money billed but not collected, including instalments on treatment plans. Track the total and the age of the oldest unpaid amounts.
12. Package and treatment plan progress
For clinics selling packages, track sessions sold versus sessions completed, and advance money held for future sessions. Unused sessions are a patient-retention warning and a liability at the same time.
Cost and profit KPIs
13. Expenses by category
Rent, salaries, commissions, consumables, utilities, generator fuel and marketing. Recording expenses in categories every week (not at month-end from memory) is what makes the profit number trustworthy. See clinic expense tracking and profit reports.
14. Staff cost as a percentage of revenue
Salaries, commissions and incentives divided by revenue. If this rises month after month while patient numbers stay flat, the schedule or the pay structure needs a review.
15. Consumable cost per procedure
For procedure-heavy clinics, the stock used per procedure (fillers, threads, dental materials, laser consumables) decides the true margin. Recording stock usage against visits makes this possible.
16. Net profit per month
Collected revenue minus all expenses. Compare it with the same month last year to remove seasonal effects such as Ramadan and wedding season.
A simple weekly KPI routine
- Monday, 15 minutes: new patients, no-show rate, revenue last week, outstanding payments.
- Monthly, one hour: revenue per doctor and service, expenses by category, staff cost percentage, net profit, returning rate, follow-up completion.
- Quarterly: pricing and package review, doctor schedules, marketing channels by new-patient source.
- Pick one KPI to improve each month and write down the action taken, so the next review shows whether it worked.
Where the numbers come from
KPIs are only as good as the data behind them. If appointments, bills, discounts and expenses live in separate registers, calculating these numbers takes hours and is often wrong. Clinic management software that records the appointment, visit, bill and expense once can show most of them on a dashboard. When choosing software, ask to see the actual reports with sample data rather than a list of report names.
How CareTrack helps owners track KPIs
CareTrack by CreativeIn Tek includes a dashboard with revenue trends, appointment overview including no-shows, patient growth, top services, payment status and outstanding amounts, plus doctor-wise figures, treatment plan reports, expense tracking by category and payroll with commissions. Data comes from the same appointments, visits and bills staff already record, on the web, the staff mobile app or the offline Windows desktop app. For a broader buying checklist, see the guide to clinic management software in Pakistan.
Frequently asked questions
What are the most important KPIs for a small clinic?
Start with five: new patients, no-show rate, revenue, outstanding payments and net profit. Add the rest as the clinic grows.
How often should clinic KPIs be reviewed?
A short weekly check and a deeper monthly review work for most clinics in Pakistan.
What is a good no-show rate?
It varies by specialty and city, so the most useful benchmark is the clinic's own trend. Aim to bring it down month by month.
Can KPIs be tracked in Excel?
Yes, for a small clinic with disciplined data entry. It becomes slow and error-prone with several doctors. See Excel vs clinic management software.
